Optum Managed Transplant Program

Optima Health will utilize Optum’s Managed Transplant Program (MTP) of care management services for all organ and blood/marrow transplants for Optima Health Commercial and Optima Family Care plans.

The transplant network will consist of Optum’s Centers of Excellence Facilities, Children’s Hospital of the King’s Daughters, Sentara Norfolk General Hospital, and Virginia Oncology Associates only.

Optima Health Case Managers will continue to be the first point of contact for new referrals and will work with Optum Case Managers to transition care management services. Providers should refer members to Optima Health Case Management at the earliest point a transplant option is considered.
