If your loved one has both Medicare and Medicaid—they may be eligible for this D-SNP—$0 monthly premium.

Optima Medicare Engage – Diabetes and Heart (HMO C-SNP)

Our Chronic Condition Special Needs Plan (C-SNP) supports members living with diabetes, congestive heart failure, or cardiovascular disease. Members receive personal attention from a care coordinator along with a team of high-quality doctors, specialty providers, and healthcare facilities to attend to their unique healthcare needs—$0 monthly premium.
