This Dual-eligible Special Needs Plan is designed for those with limited income, offering personal attention from a care coordinator as well as access to our high-quality network of more than 35,000 doctors, specialty providers, and healthcare facilities. If your loved one has both Medicare and Medicaid—they may be eligible for this D-SNP—$0 monthly premium.
Optima Medicare Engage – Diabetes and Heart (HMO C-SNP)
Our Chronic Condition Special Needs Plan (C-SNP) supports members living with diabetes, congestive heart failure, or cardiovascular disease. Members receive personal attention from a care coordinator along with a team of high-quality doctors, specialty providers, and healthcare facilities to attend to their unique healthcare needs—$0 monthly premium.

Medicare Advantage Plans
Our Medicare Advantage (Part C) plans include everything offered by Original Medicare (Part A and Part B) plus more. All plans include dental, vision, and hearing allowances. Monthly premiums from $0.
Optima Medicare Salute (HMO)
The Salute plan serves those who already have prescription coverage.
Veterans can keep Department of Veterans Affairs (VA) or TRICARE for Life benefits with prescription drug coverage while taking advantage of extra benefits—at no extra cost—$0 monthly premium.
